Interstate class application to import or export donor material – for assisted reproductive treatment (ART) providers

This application is intended for ART providers applying to import or export donor materials (i.e. donor eggs, sperm, or embryos made using donor eggs or sperm) into/out of Victoria on behalf of a class of applicants.
